Short - Term Post Hospital Discharge Care
We provide short-term care for individuals who are in-patient or transitioning home post-operatively. This service includes:
We understand that during hospitalization and immediately after discharge that additional care and assistance may be needed. Our caregivers are available to be on-site during hospitalization or an inpatient stay, at the time of discharge, and even at home immediately after discharge to assist with a smooth transition.
